+ BACKGROUND, CHALLENGES AND OBJECTIVES
Most maternal and neonatal deaths are preventable. Hypertension during pregnancy (29%) and bleeding (26%) are the two main causes of maternal death in East Java. In addition, there was an increase in the proportion of other causes of death, such as heart and cardiovascular disease, which accounted for 12% of maternal deaths. Meanwhile, the biggest causes of neonatal death are low birth weight (LBW) (42%) and asphyxia (25%). Early detection and quality treatment can prevent maternal and neonatal deaths due to this condition.
For this reason, one solution to improve the quality of maternal and neonatal services in Sidoarjo Regency is to improve the recording, reporting, and referral system, in this case encouraging the use and utilization of SiCantik application.
In Sidoarjo Regency, the implementation of a local solution “Optimizing the Use and Utilization of SiCantik” to enhance the existing ICT platform to strengthen the maternal and neonatal referral system through early detection of complications and timely referral for high-risk pregnancies.
Efforts to optimize the use and utilization of the SiCantik application include facilitating the issuance of regional regulations in the form of Regent Regulation number 27 of 2020 about the Implementation of Maternal and Child Health Services (KIA); which was made in a participatory manner involving various elements of the health and non-health sectors in Sidoarjo Regency (Penakib Forum, IBI, Academics, Hospitals, Private Hospitals, related OPD, Public Health Centers, the private sector, and the media).
In general, the purpose of making SiCantik is to accelerate the decline in ICT-based AKI and AKN (Information, Communication, and Technology), while the specific goals/targets of the existence of the SiCantik application are:
1. Every pregnant woman, mother giving birth, and newborns and toddlers receive services according to standards, to support the achievement of indicator coverage in the Minimum Service Standards (SPM); Then it was developed with an application for the health of prospective brides (Catin) as an upstream target in accelerating the decline in AKI AKB and stunting toddlers, further application development to school-age children's health and adolescent health and elderly health.
2. Documenting; maternal and child health data;
3. As a communication tool between health workers related to maternal and child health;
4. Facilitate the recording, storage, and retrieval of data on pregnant women as well as early detection of high-risk pregnant women in the Sidoarjo Regency area;
5. Tools to make emergency referrals or planning referrals to high-risk pregnant women; and
6. Improve coordination with primary and secondary health care facilities in maternity services.
+ ACTIONS AND IMPLEMENTATION
To optimize the use and utilization of SiCantik include facilitating the issuance of regional regulations in the form of Regent Regulation number 27 of 2020 about the Implementation of Maternal and Child Health Services (KIA) which was made in a participatory manner involving various elements of the health and non-health sectors in Sidoarjo Regency (Penakib Forum, IBI, academics, hospitals, private hospitals, related OPD, Puskesmas, private sector, and the media). For more optimal use and utilization of the SiCantik application, the preparation of guidelines and SOPs for its use has been facilitated so that health workers and health service facilities are easier to use.
Evaluation of SiCantik is carried out internally by the internal institution, namely evaluating the use and development of applications to support data collection in KIA services so that SiCantik can produce appropriate results. The evaluation will recap the number of active users or medical personnel entering data. The recap user data that does not optimize SiCantik both from internal and external can be evaluated properly.
Evaluation of SiCantik is carried out by external institution or related stakeholders, namely evaluating the use or utilization of SiCantik and discussing obstacles or difficulties in implementing SiCantik.
Strategies to optimize the use and utilization of SiCantik are carried out so that innovation continues, namely by facilitating the issuance of regional regulations in the form of:
1. Regent Regulation number 27 of 2020 concerning the Implementation of Maternal and Child Health Services (KIA) and
2. SiCantik Application Manual and SOP.
With the regulations, guidelines, and SOPs for SiCantik, will increase usage so that innovation will sustain. In addition, in every socialization activity or evaluation of SiCantik, an agreement is made for a warning or sanction for users or medical personnel who do not optimize this innovation.
+ OUTCOMES AND IMPACTS
The success of the innovation is that health workers fill in data in the SiCantik application because the application data is obtained from health services carried out by health workers. The data entered will obtain K1, K4, KF, and KN data so that monitoring and early detection of high-risk pregnant women can be carried out. The benefits of SiCantik are felt by monitored pregnant women, health workers, and the person in charge of MCH in relation to reporting health data.
Success can be measured by the achievement of data that has been entered by the SiCantik application and the level of safety for pregnant women and babies or health workers who serve.
In the 2018, there are 1632 pregnant women with 3330 visit. Then, in the 2022, there are 10661 pregnant women with 23333 visit. This progress shown that SiCantik is succesfully record and monitor pregnant women in the Sidoarjo Regency.
The obstacle in the process of using innovation is that the use or utilization of the SiCantik application in entering data is not optimal so the results of the data or evaluation do not reach the expected.
+ REPLICABILITY AND SCALABILITY
SiCantik has the potential to be an example for agencies or regions because the innovation applies technology-based services that will make it easier in various aspects such as recording, storage, and others so that they can solve problems properly and quickly. The application of innovation which is carried out by other agencies or regions will be adapted and bridged so that it will obtain larger data and cover all matters or problems.
+ BUDGET SOURCE
The resources used in carrying out innovations, namely the budget or finance used are the Sidoarjo district government budget which then the budget funds are allocated to providers as human resources for the development of the SiCantik application. Meanwhile, to support innovation, an internet connection, server, and several computers are needed as server managers and application admins.
